I don't think it's that they don't understand... it's that they believe the behaviours and standards of society don't, or shouldn't, apply to them and have no empathy or conscience regarding others. they know very well how to imitate for personal gain, however, so I think understanding is not the issue.
AYRT: It is born from not understanding, relating or getting / feelling appropriate emotions which can be self-learned in an absolutely healthy way but the disorder part comes where it is not and goes the way you describe - you see society as an opponent in an endless game against you and the only way you can win is to play their game according to your own rules (at least that is the reasoning I had and it is as self-centred as it sounds). The hard bit is keeping yourself in check without the applause of your audience which you actually loathe (Andy fits the bill in that).
You probably phrased it better than me though as I meant it is an overtly self-aware disorder which actually can be maintained in same way as empathy is actually learnable, same as anything else.
